We collect mobile phone numbers to communicate with patients via SMS or RCS text messages for purposes such as appointment reminders, billing notifications, and care coordination. We will not send unsecure messages containing protected health information (PHI). We do not sell or share your phone number with unaffiliated third parties. You may opt out of receiving text messages at any time by replying “STOP.” Message frequency may vary. Message and data rates may apply. For help, reply “HELP” or contact us at (833) 280-2871 and/or website@spireortho.com.
Changes To This Notice
We reserve the right to change this Notice. We reserve the right to make the changed Notice effective for Protected Health Information we already have as well as for any Protected Health Information we create or receive in the future. If we make a material revision to this Notice, we will promptly publish the revised Notice on our website and will distribute the revised Notice to all current patients.
Complaints
If you feel that your privacy has been violated, please notify the Chief Compliance and Privacy Officer, Karyn Wilkinson via our Spire Orthopedic Partners Compliance Helpline by phone at (833) 280-2871 or online at spireortho.ethicspoint.com.
You may also file a complaint with the Secretary of the United States Department of Health and Human Services. All complaints must be made in writing and should be submitted within 180 days of when you knew or should have known of the suspected violation. There will be no retaliation against you for filing a complaint.
