Insurance

 

We participate in most insurance and HMO plans available in the area. As a service to our patients with insurance, we will file all claims on behalf of the patient. In order to file claims properly you will need to provide our office with complete billing information, including mailing addresses and ID/certificate numbers. If you are involved with a managed care (HMO) plan, please be sure to follow the guidelines of your plan. We ask you to contact your PCP at least a week in advance to provide any referrals that are needed. Without a referral, you will be responsible for the entire bill.

If you are injured at work, we will submit your bill to the Worker’s Compensation carrier as long as all the proper information is provided to us. Information needed is the WC carrier’s name and address along with date of injury and claim number. Without this information we are unable to bill the WC insurance and you will be responsible for the entire bill.

If you are injured in an auto accident and have med pay benefits on your auto insurance we will gladly submit to them until the med pay benefits are exhausted. Once the benefits are exhausted we will then submit the claims to your primary insurance at which time you will be responsible for any copays or deductibles.

We DO NOT bill third party insurances. We cannot hold another party responsible for payment of services that you receive. This applies to insurance companies, as well as to other parties, who you believe may be responsible for payment of your bills.

Payment Policy

All copayments, co insurance and deductibles are expected at the time of service. For your convenience VISA, MasterCard, and Discover are accepted in addition to cash and checks. You will receive our financial agreement form, please review the information as you will be asked to sign at the time of your first visit.
