In orthopedics, two of the most widely used injectable treatments are cortisone injections and gel injections (viscosupplementation). While they work through completely different biological mechanisms, both have the potential to significantly improve your pain and mobility. To help you make an informed decision alongside your care team, let's break down the details, benefits, and trade-offs of each option.
Cortisone Injections
Cortisone is a synthetic corticosteroid that mimics the anti-inflammatory hormones your body naturally produces to manage stress and inflammation. When administered directly into a joint at therapeutic doses, cortisone works by blocking the chemical pathways that trigger heat, redness, swelling, and pain.
One of the main advantages of cortisone is how quickly it works. Many patients notice a significant drop in symptoms within 24 to 48 hours after administration, with relief lasting anywhere from several weeks to several months depending on the individual. Furthermore, cortisone is readily available in most orthopedic clinics and can often be administered on the same day as your visit, making it an ideal choice for acute flare-ups of pain.
Cortisone does not structurally repair chronic cartilage wear and tear. Instead, its primary goal is to decrease inflammation enough to allow for return to daily activities and active participation in physical therapy to strengthen the surrounding muscles. This can make it a beneficial option for patients who are not ready for or healthy enough to undergo surgery.
However, frequent administration comes with trade-offs. Overusing cortisone can lead to soft tissue breakdown, cartilage softening, and localized tissue weakening. Cortisone can also suppress the immune system, creating a slightly increased risk of joint infection. For these reasons, your healthcare provider will typically limit your treatment to three or four times per year depending on your clinical scenario.
In certain situations, cortisone may not be indicated. In patients with diabetes, cortisone administration can temporarily raise blood sugar levels. Steroids can also impair bone healing, so they are generally not recommended for joints that have sustained recent fractures. If a patient has experienced a severe allergic reaction to cortisone in the past, proceeding with additional injections is not advised.
Gel Injections
Gel injections, also referred to as viscosupplementation, utilize hyaluronic acid to provide lubrication directly to the joint to decrease pain rather than focusing on anti-inflammatory pathways.
Currently, viscosupplementation is only FDA-approved for the treatment of knee pain. However, healthcare professionals are permitted to use these injections off-label for other joints, such as hips or shoulders, if they determine it is clinically appropriate.
Healthy joints are filled with synovial fluid, a natural substance rich in hyaluronic acid that the body uses to decrease friction and shock on cartilage. In worn or arthritic joints, this fluid thins out. Injecting synthetic hyaluronic acid cushions the joint and can also stimulate the joint lining to restart its own natural fluid production over time.
There are multiple brand options available on the market, which may be chosen based on cost, insurance coverage, and preference. Based on the specific formulation, the administration schedule may range from a single one-time injection to a series of three injections administered once a week over a three-week period.
Gel injections do not provide as fast-acting relief as cortisone. Most patients begin noticing improvement around three to four weeks after administration because the mechanism focuses primarily on stimulating fluid production and restoring lubrication rather than immediately blunting pain signals.
Like cortisone, gel injections can provide long-lasting relief and serve as a helpful tool for patients looking to delay surgery or manage pain while working on physical therapy. Also like cortisone, these injections do not fix or reverse structural wear and tear within the joint.
Gel injections are generally considered the more benign of the two options regarding side effects and contraindications. They do not cause spikes in blood sugar and have not been shown to cause cartilage degradation. While patients with egg or poultry allergies are advised to exercise caution with certain formulations, many modern gels are synthetically produced and are safe for these individuals.
Unlike cortisone injections, viscosupplementation is not kept in the clinic for same-day administration and must be ordered specifically for each patient. In most situations, healthcare providers can submit for insurance approval for these injections up to twice a year.
