You will receive confirmation of your appointment via text with a link to fill out your patient intake forms. To make check-in easier with our new Electronic Medical Record (EMR) system, please download APPatient and complete your forms before your visit. Visit orthori.com/portal to download the app.
If you haven’t filled out your patient intake forms and medical history on the APPatient app, we will provide a tablet during your visit to complete these forms.
If you use APPatient for contactless check-in, please let the front desk know when you arrive at the office.
Our required patient intake forms are made available online. This allows you to print and complete them before your appointment.
